I'm about 3/4 through this game.  I'm enjoying it thus far.  Music is great, as is the scenery.  They let you adjust the difficulty in the options menu so if things get too tough or too easy you can adjust it.  Some of the puzzles frustrated me as did swinging between the levels.  I just took a break and came back to it later and it just clicked.  It's not a perfect game.  But it is fun.

I played it through the second time and maxed my out unholy side to get the achievements. I also got the Dark Forest DLC and it gives you a Disco suit for Dante.

As far as gruesome, it's pretty good monster wise. You can't think of the babies are real, they are game enemies. They have knives for hands and if you don't kill them, they kill you.

You can also change the difficulty in the menu screen, I did that to test it. The hardest level to unlock is Inferno and I doubt anyone could win on that. It's insane.
